Colleague and Patient Feedback Surveys
Multi-source feedback is an essential part of providing medical services. At Osiris MR we have everything you need to gather vital data from colleagues and patients, to ensure you’re providing the best possible service.
Our package has three parts. The first is the Patient Questionnaire (PQ) – a paper questionnaire which patients complete after their consultation, to provide honest, anonymous feedback about the quality of care they have received. The second is the Colleague Questionnaire (CQ) – an online questionnaire in which practitioners provide feedback about the service provided by their co-workers. The third is the Self-Assessment Questionnaire (SQ) – another online questionnaire in which practitioners evaluate their own performance by answering the same questions as those put to their colleagues and patients.
Osiris MR will take care of the legwork for each part of the survey. For the patient questionnaire we will provide paper surveys, envelopes, and a ballot box. For the colleague questionnaire we will invite your pre-selected participants via email to carry it out, as well as keeping track of how many have been completed and sending out reminders for any that are outstanding. Once the survey is complete, we will provide a detailed report of what patients and colleagues have said, where improvements can be made, and how what they have said compares to what practitioners said in their self-assessment questionnaires.

Our Patient Questionnaire is a paper survey given to patients after their consultation, allowing them to provide honest, comprehensive and anonymous feedback about the quality of care they have received.
We will furnish you with everything you need to complete the survey – as well as the paper surveys themselves we will also provide envelopes, a ballot box, and detailed instructions to guide you through the survey process
Like patient feedback, colleague feedback is essential both to meet GMC guidelines and to ensure you are providing the best possible service to your users.
Our Colleague Questionnaire is a secure online survey. Once you provide us with a list of colleagues to assess, we will invite them via email to provide honest, anonymous feedback about their colleagues.
As well as this, we will keep track of how many have been completed and send out reminders where necessary.

Ours is an online questionnaire in which participants rate themselves against the questions in the patient and colleague questionnaires. This allows you to see how practitioners feel they are performing and, more importantly, to compare their perception to that of their patients and colleagues.
GP surgery questionnaires

Our questionnaire will take your patients’ feedback concerning all aspects of their visit: the care they received from the practitioner, the interpersonal skills of the staff, how long they waited, the condition of the waiting room etc.
We can provide a report both on the practice as a whole, and of the individual practitioners, to ensure you are providing your patients with the best possible care on an individual and an institutional level.
Additionally, we can offer a Friends and Family Test (FFT). This is an NHS initiative that provides even more valuable data, by asking how likely your patients are to recommend the practice to their friends or family.
